Pathways offers a complete family of health services to help you manage care at home, assisted living facility, nursing home, or senior living community.

Pathways Hospice


Hospice care is for anyone, with any health condition, whose doctor thinks he or she probably has six months or less to live.  We make it possible to stay at home during one's final months or weeks.

Pathways Grief Support Services


Pathways offers the following bereavement services at no charge to the community:  individual counseling, support groups, workshops, mailings, phone calls and visits, memorial services, and remembrance events.

Pathways Home Health


Home health is a wide range of health care services that can be given at home.  It's intended for people who have difficulty leaving home to get care.  This is called home bound.

Pathways Private Duty


We also can assist older adults and the chronically ill in the activities of daily life.  This can be temporary or on a long-term basis, up to 24 hours a day.  Care can be provided in the home, hospital, skilled nursing facility, or assisted living facility.

Pathways Hospice Foundation


A philanthropic program at Pathways, our foundation helps us care for patients and families at all stages of life.  Pathways helps handle a health crisis, cope with chronic illness, find comfort in the final stages of life, or heal from the pain of loss.  Every gift made to Pathways makes a difference.

  5. My grandmother is getting older, and I don't think she can keep living on her own any more. I go over and visit several times a week, but I think it's time for her to get some extra care. I can still visit her, but she'll be able to get more help when I can't be around.
